New Patients: What To Expect

Welcome to our practice! If you are one of our new patients, please arrive 15 minutes prior to your appointment to fill out the necessary medical history and insurance paperwork, or, download the forms here and complete them at your convenience. Simply bring them with you to your appointment. You will be asked to present your medical insurance card, your vision insurance card. Plan on spending about an hour and a half if you are being seen for a comprehensive eye examination. A comprehensive eye exam includes the use dilating eye drops to detect eye problems such as glaucoma or cataracts, as well as to check for systemic conditions such as diabetes. These drops may also blur your vision for a short time after your exam.

Depending on the nature of your visit, our staff will submit your insurance claim to either your vision plan or your medical insurance. We are required by insurance laws to bill the carrier that most accurately reflects the nature of your symptoms and diagnosis. Only well-vision exams for glasses or contact lenses are covered by vision plans. Examinations in which a health problem is detected or treated must be billed to your medical insurance. In order to help control health care costs, please be prepared to cover charges not covered by your plan at the time of service including all co-pays and deductibles.
